UB-04 laser-cut forms are designed for medical offices to file a claim with the patient's insurance company. The Health Care Finance Administration format ensures accuracy in reporting all necessary information. Forms meet the requirements of the Centers for Medicare and Medicaid Services (CMS). Forms are printed on 20 lb. bond paper.

Guest Check Book ensures proper documentation of important sales information on carbonless forms. Ruled chart offers a place to itemize an order and to list quantity and cost of each item. Detail up to 11 items. Each form also has space for the total cost, name of waiter, number of persons and table number. Two-part forms are consecutively numbered. The original is printed in blue and green ink, and the duplicate is printed in red ink. Detached size is 3-11/32" x 6-3/8".

Voice Message Log Book features a flash message format that helps you keep an accurate record of your voice messages while on-the-go. The cover of the spiralbound book flips back for easy writing in a vehicle. Each page includes 14 entries. Each sheet is printed on both sides for a total of 1,400 messages per book. Each entry includes a checkbox and four lines for the date, name/number and notes.

Two-part format includes an original in white with blue and red ink, and a canary duplicate with a black/white image. Stop card prevents write-through to sets on the next page. Each form includes a place to write the date, payer, amount paid, purpose of payments, time period that the payments covers, account balance, remainder due on the account, and boxes labeled cash, check and money order to choose payment method. Wirebound book opens flat and contains 200 sets per book.
