The two-page-per-week format has 21 lines to a page and columns for each day of the week, even weekend days, a spot to fill in the week-ending and the person who approved the time worked.

Each page features columns for date, name, address, time, badge number, company and address, citizen, department or person to visit, purpose of visit, time in/out and remarks. Use for factories, warehouses and large offices where a detailed record of callers must be kept.
