Manila weekly time cards are equivalent to cards from time clock manufacturers. Each time card features the names of the days printed on the card. Left side of card is for extra time; right side is for regular time. The front is printed in green ink. Time cards replace Card 10-800292, M-33, 33, 1950-9305.

Equivalent to time clock manufacturer's time cards, these time cards feature a job time record used for accurate job costing. Boxes keep track of job, amount, rate, elapsed time and total time worked. 14 lines let you track an employee's time worked at different jobs. Extra-heavy, 143 lb. manila stock is printed on one side in green ink. Time cards replace L-61, 1950-9502, 15-80062, and are designed for use with Acroprint 125R, 150R, ES700, ES900, ES180, 175TR; Cincinnati 1000, 10000; Lathem 900E, 1200, 1500E, 2000, 4000, 5000EP; Simplex KCF, KCG, JCG, JCP and 2500.

Weekly time cards for Pyramid 1000 and 2000 feature a Monday-through-Sunday format, overtime space and full payroll calculations. Front has the named days of week. Back offers space for overtime hours and weekly wage deductions. Both sides are printed in green. Time cards replace 331-10 and are made of extra-heavy, 143 lb. manila stock.

UB-04 laser-cut forms are designed for medical offices to file a claim with the patient's insurance company. The Health Care Finance Administration format ensures accuracy in reporting all necessary information. Forms meet the requirements of the Centers for Medicare and Medicaid Services (CMS). Forms are printed on 20 lb. bond paper.
